EpiPen and EpiPen Jr Auto-Injectors - Important Safety Information on Correct Usage - Pfizer Canada Inc.

Health Canada is informing Canadians of Pfizer Canada’s instruction for the correct use of EpiPen and EpiPen Jr., particularly the correct way to remove the blue safety caps.

I read the advisory, and wonder if it is a design flaw? In a panic situation, pulling the blue top off at an angle, in my opinion, should not adversely affect the operation of the epipen.

If pulled off incorrectly the thin 'inner' tube can potentially be broken and remain inside the pen effectively meaning it is not ready to fire. (when you pull off the blue end cap it has a thin middle bit that has to come out of the epipen before it can operate correctly-twisting the cap off can potentially break part of this off inside the epipen).
